Retatrutide

Also known as Triple agonist LY3437943, GLP3-R

Metabolic
EOL-005

Structure

36 amino acids

Half-Life

6 days

Origin

Eli Lilly

Research

Phase 3 TRIUMPH-3 ongoing

What it is

A triple receptor agonist activating GLP-1, GIP and glucagon receptors simultaneously. The most mechanistically complex incretin compound under active investigation. Phase 2 TRIUMPH data showed 24 percent mean weight reduction at the highest dose.

Mechanism of Action

GLP-1R for appetite suppression and insulin secretion. GIPR for insulin sensitisation and adipose lipid metabolism. Glucagon receptor increases basal metabolic rate, promotes hepatic fat oxidation and activates thermogenic brown adipose tissue.

In Plain English

Each generation hits one more receptor. Semaglutide, one receptor: eat less. Tirzepatide, two receptors: eat less and handle insulin better. Retatrutide, three: eat less, handle insulin better AND burn more calories at rest. Phase 2 showed 24 percent average weight loss. Nothing approved today comes close.

What researchers measure

  • Percentage body weight reduction
  • Resting energy expenditure
  • Hepatic fat oxidation
  • HbA1c change
  • Brown adipose activity markers
  • Lipid panel shifts

Research has studied

  • Obesity TRIUMPH Phase 2 and Phase 3 programme
  • Type 2 diabetes glycaemic endpoints
  • Hepatic steatosis reduction
  • Energy expenditure and thermogenesis
  • Insulin sensitivity
  • Cardiometabolic risk markers

Origin Story

Developed by Eli Lilly as the successor to tirzepatide, adding glucagon receptor activity to the dual incretin backbone. Currently in the Phase 3 TRIUMPH programme.

Best suited for

Men and women 30-65 pursuing advanced fat loss, highest potency metabolic support, and those who have plateaued with other GLP compounds.

Usage notes

  • Reconstitute with sterile or bacteriostatic water.
  • Add diluent slowly down the vial wall.
  • 6-day half-life, model accumulation across weekly protocols.
  • Store reconstituted at 2-8C, lyophilised at -20C.
  • Protect from light.

Key considerations

  • Glucagon receptor activity raises metabolic rate, a distinct mechanism.
  • Highest reported mean weight reduction in the incretin class.
  • Still investigational, Phase 3 data pending.
  • Dose escalation is central to tolerability in published trials.
